INTERPROFISSIONALITY IN PRIMARY CARE IN THE CONTEXT OF SOCIAL VULNERABILITY: AN EXPERIENCE REPORT.

Abstract

In Brazilian scenery, the Health’s Primary Attention acts as care’s coordinator from dynamic understanding of the social determinants of human life, being fundamental the emergence of a professional profile that understands these problems by interprofessional practicing.  Thus, the Education by Working Program (PET in Portuguese) emerges to strengthen the teaching- service- community experience, and establish links with the territory and assisted population. The purpose of this work is relating the PET’s monitors experience in a Family’s Health Center (CSF in Portuguese), located by huge social vulnerabilities. By the scholar insertion in the reference center it was possible have a broader view on the population’s health conditions and identify the various local problems. For this, activities were planned, such as home visits which was possible to fully understand the health and disease process of the studied population, in addition to highlighting the importance of teamwork by different professionals to increase the capacity for perception and intervention on health. Therefore, the CSF is an important tool for mitigating the social vulnerabilities present in the territory, despite having weaknesses with regard to the fragmentation of work.
